Reminder for Participants:

Important: After your enrolling site completes its participation in the trial, all long-term research follow-up will be managed centrally by Stanford University and the University of Alabama at Birmingham (UAB). Your regular medical care continues with your own provider. This will be explained fully during the consent process.
